Tragic that a person with a minor injury lost her life due to what appears a lack of training.

That's one of the first things we teach in divers & rescuers of waterbourne transports of prisoners & victims.

Don't tie or handcuff anyone to the vessel & place them in a PFD.

And a properly rigged Stokes will float upright with the victims head out of the water.
